Job overview
We have an exciting opportunity for a Therapy Assistant Practitioner to join our Ipswich East Integrated Neighbourhood Teams (INT).
This post is based in the centre of Ipswich at a co-location with the Ipswich West INT, REACT and pathway 1 clinical teams.
The successful candidate will work as part of the therapy teams, providing therapy to patients at risk of admission to hospital, while also supporting the Urgent Care Response Team
The successful candidates will work across 7 days a week, and extended hours as part of the INT therapy rota.
Main duties of the job
The role will allow the successful candidate to manage a defined caseload of patients in the community under the guidance of a registered therapy clinician.
The Therapy Assistant Practitioners will with support from a registered therapist respond to patients therapy needs in their own homes to prevent Hospital admission and support the wider Urgent Care Response to prevent Hospital admission through crisis.
This role will be a valued, key member of the multi-disciplinary team carrying forward treatment plans and progressing rehabilitation goals for patients to enhance recovery and allowing them to remain in their own home.

Experience
Essential criteria
Previous experience in the NHS
Experience as a Therapy assistant or experience at a similar level in a related field of health care / health and fitness sector
Desirable criteria
Knowledge of community resources and whole systems/ pathways approach to care
Qualifications
Essential criteria
Can demonstrate intermediate level of theoretical knowledge and clinical reasoning skills acquired through coursework, in-house training and experience to NVQ/Diploma Level 3 or equivalent
Desirable criteria
Foundation degree
Membership or special interest in a community care and integration
Assistant member of Professional group such as CSP or BAOT or working towards this
Skills
Essential criteria
Car owner/driver/access to a motor vehicle (role will involve travel to other sites that cannot be easily accessed by public transport means)

IT literate
