Job overview
An exciting opportunity has become available for a Band 5 Registered Nurse to Co-ordinate Discharges at Priscilla Bacon Lodge – a Specialist Palliative Care Unit. This post will be 21 hours a week, working Wednesday's, Saturdays and 1 other week day which can be decided at interview.

Main duties of the job
The post holder will:
Coo-ordinate discharges of patients admitted to the Specialist Palliative Care in-patient unit.
Liaise closely with Health and Social care professionals across Norwich, South and North localities to ensure smooth and timely transfers of care and discharges based on the patients needs and choice.
Work with the Multi-disciplinary team, identify the need for development in the present processes and proactively work toward improving flow of patients and preventing delay in discharge.

Detailed job description and main responsibilities
To facilitate discharge of patient ensuring high quality holistic individualised care to patients and their families.
To ensure patient centred and clinically effective service.
To participate in developing joint action plans, protocols and pathways and influence the use of these.
To work in line with protocols and pathways with appropriate information which prioritise palliative care patients and address the particular needs of the patient.
Work closely with Healthcare professionals in continuing care to design appropriate packages of care.
To liaise with ward and pharmacy staff regarding organisation of TTOs.
To attend MDT handovers.

Essential criteria
- Current Nursing registration
Experience
Essential criteria
- Experience of working within a multidisciplinary team.
- Experience of working with patients and developing and delivering care plans designed to meet their needs.
- Experience in arranging packages of care to facilitate discharge
Desirable criteria
- Experience in Continuing Health Care Assessments
Skills
Essential criteria
- Excellent verbal written and interpersonal communication skills
- Basic Computer skills
- Ability to maintain and improve standards of care
