Job overview

An exciting opportunity has arisen within the Ipswich Community  Team. We are looking for two experienced band 6 Physiotherapists who are passionate, committed, innovative, enthusiastic and highly motivate to delivering high quality care within patient’s homes.

There are two full-time positions one in our Ipswich East Team and one in our Ipswich West Team.

We are fortunate enough to be co-located with our nursing, social care colleagues and the out of hours REACT, admission prevention and discharge to assess teams.

This is an opportunity to work within a knowledgeable experienced team and wider MDT. Candidates should be flexible, motivated and willing to work within a changing environment. We will provide support through in-service training and professional forums, Mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators. We facilitate students on training and have close working relationships with neighbouring community teams in Health and Social Care.

 

Main duties of the job

We are committed to patient centred care in the right place, at the right time and provide a comprehensive supervision and training programme with career progression opportunities within the clinical and managerial fields. 

The Therapy service has a high profile within the Trust operating a 7-day service and thrives on innovation and challenge. It has taken a lead role in many developments, which are helping to shape the future of patient pathways within the local area.

Main Responsibilities:

To provide high standard patient care in hospital settings and patients homes

To ensure a more seamless flow for patient care between hospital and community settings, enabling patients to be cared for in their own home environment 

To provide supervision and support to more junior staff

To triage referrals to the community team

Essential criteria

Evidence of skill relevant to the rehabilitation of adults with physical disabilities gained through theory and practical course and experience.

Community Experience

Evidence of crisis management, admission avoidance and supporting discharge from acute and community hospitals

Joint working across health and social care services

Able to travel to rural locations.

Desirable criteria

A specialist knowledge and application of current best practice in the rehabilitation of adults with physical disabilities
